Main Office is open from 9am to 4:45pm Monday-Thursday and from 9am to 4pm on Friday. Sister Location is open from 8:30am to 4:45pm Monday-Thursday and closed on Friday. Phones go to answering service 12:30-01:30 daily.
We do not schedule, cancel, or re-schedule appoints via email. We do not handle medication refills or any other inquiries regarding your treatment via email. Please call the office.
Please use the back door to enter our main office. This door is not visible from Magnolia Avenue.
If you are an existing patient and would like to request an invitation to open a portal account please contact our office and provide us your email address.
All forms must be uploaded to the portal once you receive access.
If you are a new patient and do not complete your paperwork in advance through the patient portal please arrive 30 minutes prior to your appointment to complete it.
